The church is in the process of converting our paper records of individuals and families of our congregation, into a digital format. Much of the information we have on file has been entered into the database file but the contact information for many families and individuals is out of date. If you wish for your family to be listed in the directory we ask that you download the form below and provide your current contact information.
Please complete all fields of information that pertain to you and your family members.
The directory, depending upon your preference, will include your mailing address, phone #s, names, & email addresses of members of your family, wedding anniversary & birthdays. Birthdays and anniversary dates that you provide could be used in the directory or weekly church bulletin, enabling the congregation to celebrate these events in your family. The day and month would be printed but not the year. You may indicate any information that you wish to keep private and it will not be published.
If you have children 18 or older, they may wish to have their own individual listing in the directory. Just have them fill out a separate form.
Pictures will be included with your family listing. You may submit your photo by email or drop it off at the office.
Finally, if you DO NOT wish to be listed in the directory please complete the form anyway. The information that you provide will be used to maintain internal church records. Complete the form and indicate that you should not be listed in the directory.
